Health Assessments Offered:
4 Year Old Healthy Kids Check & Childhood Immunisations
The Healthy Kids Check aims to gather health information, identify health problems and promote healthy lifestyles around the time of the 4 years of age vaccinations. It consists of a checklist of examinations and assessments, six of which are mandatory.
45-49 Year Old Health Assessment
A one-off health assessment to people between 45-49 years of age. It is designed to identify health risk factors, and assist in the prevention or early diagnosis of chronic conditions, such as diabetes.
Over 75 Health Assessment
A comprehensive annual at-home or in-practice health assessment designed to help older Australians stay independent, safe and confident about their health.
Aboriginal and Torres Strait Islander Health Assessment
Available annually to all people of Aboriginal and Torres Strait Islander descent, of all ages.
Comprehensive Health Assessment Program (CHAP)
Designed for children and adults with a disability. The assessment may include an assessment of your teeth, hearing, eye sight, nutrition, any
swallowing problems, immunisations and so on. Your nurse and GP will advise on preventative health care, treatment and any other medical tests that are needed.

Our new patient form captures important demographic information to ensure that your health record is up to date, and in order for us to provide you with the best care. Please fill this out if you are visiting our practice for the first time.
Prepare for Your First Visit: Please bring a list of medications and supplements you are currently taking, along with any relevant medical records (discharge summaries/ test results etc.) This will help the GP to understand your medical history and provide appropriate treatment.
